Before B-1 lands out-of-bounds, he passes to B-2 who is in-bounds. The Trail official sounds his whistle, rules no violation and awards the ball to B-1 for an end line throw-in do-over. Was the official correct?

The pre-game warmup clock runs out and neither team has provided a line-up for the official scorebook. The referee rules an indirect technical foul on each head coach. No free throws are attempted and the game starts with a jump ball.
During the 2nd quarter it is discovered that #21 on Team A is not in the book. #21 is added to the book and the coach of Team A is charged with an indirect technical foul. Team B is awarded two free throws and the ball at the division line for a throw-in.
The head coach of Team A is warned that another indirect technical foul will result in his ejection.
During the 3rd quarter Team A is discovered to be playing with 6 players on the court. An indirect technical foul is charged to the head coach of Team A and he is disqualified. Team B is awarded two free throws and the ball at the division line for a throw-in.

With 7:31 remaining in the 4th quarter B-1(#4) intentionally fouls A-3 during a live ball. After the whistle B-1 begins cursing out A-3. The official rules a technical foul on B-1. A-6 and A-7 leave the bench and come out onto the court by the free throw lane. No member of Team B’s bench comes onto the court. No fight takes place. Handle the situation.

The officials sound the whistle as the head coaches from both teams and the assistant coach from Team A rush onto the court to prevent the situation from escalating. Handle the situation.
